Adult patients ventilated more than 48 h
Stable respiratory and hemodynamic conditions for SBT
Consent of patients
Arterial line

Exclusion criteria

severe COBP
Laryngeal dyspnea
Tracheostomy
Arrhythmia
No echogenicity
Paraplegia >T8
